What should I do to be prepared for my office visit?
Parking fills up from time to time through the day, so please allow extra time for this. Please try and be in the building at least 15 minutes prior to your appointment time to allow time for checking in and getting to the rooms.
Please bring in your insurance card and medication list to every visit. Remember that co-pays are due when you check in prior to your visit.
If there have been any significant changes to your health, such as emergency room visits or hospitalizations since your last visit. Please let our schedulers know ahead of time so that pertinent records can be obtained and there will be adequate time in your appointment to go over these changes.
What if it's my first office visit?
If this is your first office visit, we will need your Patient Registration Form and your Medical History Form completed prior to your visit. We also would like to have your prior medical records if possible and the Medical Records Request Form will be needed to go to your previous provider for this. It is best if you can do this ahead of time, but if not, we can assist you with this at your first office visit. We will also need you to review our Notice of Health Information Privacy Practices and print out a Privacy Statement Form to sign and bring in prior to your visit.
